Family Health Center

Location Information

The Family Health Center is located across the street from White Memorial Medical Center.  In addition to our private practice, we serve as the outpatient training site for the White Memorial Medical Center Family Medicine Residency Program.

We strive to provide all of our patients – from birth (and even before) to the end of life – and their families a personalized, caring relationship and to deliver the highest quality of medical care to our community.

The Family Health Center was recently recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance. The PCMH is a nationwide effort established to provide comprehensive, culturally competent, and coordinated care.  FCS-FHC has gone through a rigorous accreditation system to achieve our PCMH designation.
